It´s a 4-stage FET-based Phaser made by colorsound.
The schematic is very similar (or even identical) to the:
Colorsound Phazex schematic

Sold by Custom Sound in the UK. I have this FX sheet from them and a price sheet dated 1977. Seems that their primary interest was in amplification and PA gear.
