Look at the netlist file which has been generated from your schematic & see what is going on around line 56. Hopefully there will be some component references which can point you to a bit of the schematic which has an error of some kind.

Looking at the pcb a bit more, looks like the weird looking cap may be due to switching circuit not being included in the schematic.

C3/C6 and C15/C4 look odd. The smaller capacitor dominates in each case & makes the larger capacitor redundant.

If the 3PDT were used to switch the supply to the anode side of the LEDs, and the bypass loop LED's cathode were connected to the ring terminal of the send or receive jacks, then the bypass LED would only light if the bypass loop is connected.

But the only way to forward-bias D2 is for pin 6 to go below GND. Even if that were possible, with the supply rails on IC3 being +Vc/GND, I don't see how the input circuitry in the opamp can work.
