I looked through my stuff and couldn't find anything. What I do is buy a 1 Meg reverse audio pot, add wires to the lugs to extend them, and cut the shaft to a D-shape with a dremel. I've done this several times over the years and have had good luck with it. With the knob on it you can't even tell.

I have a similar switching on my Royal fuzz-wah. I think the Ibanez Screaming Demon has a different kind of switching, too.

Not sure if it's relevant to this model but the plastic strip on the treadle may be broken. It goes down in a slot on the top.
