If it is a quad opamp, it might be the same pinout as rc4136, you might try and that looks plausible from tracing part of the pcb, for instance power supply connections, they go to the end if it's a similar pinout, and to the middle if it's the modern pinout.

Have you tried a different power supply (battery?)?

Was that the quad opamp with off-standard pinout? IIRC it was old but you could do a modern opamp on an adapter board to change the pinout (if there's room for it).
Maybe a place like the electronic goldmine still has them?
