I think Guillaume Fairfield has been making them (professionally) like this since 2008, but I agree that there are probably far fewer conmen amongst the successful "boutique" builders nowadays. I really like the way his pedals look like excellent quality DIY/handmade boxes from the outside with the lack of paint/stickers/silkscreen and stamped legends, but are v.professionally built as you point out.

Probably, yeah
Might solder them in, measure Hfe, and then plug the numbers in a spreadsheet to determine resistors, might be a bit of secret sauce to get all pedals on similar bias points to make sure they sound similar.

Affordable PCB fab houses have made high quality clean builds so much easier to achieve in the last decade compared to the 2000s.

Sure, and since large schale manufacturers also strived for everything on a single board, more board mount components are available.
