Mmm.. it says OD-02 so perhaps a Maxon OD-02? how many pots does it have?

Thank you for responding! The orange, red and purple group of wires goes to one pot. It has input and output wired up as well as the battery clip. The brown and purple go to nothing and the brown and black near the top (notched pcb side) also nothing. I would assume one is for the switch and one for another pot? There are also those three holes in the board next to the orange, red and purple group that I suspect could be another pot.

The op am is NEC Japan C4558C 428L

This was found in a box of NOS Boss boards and I thought it was maybe a Boss Overdrive version however it looks like that is not the case.

Thats the level (volume) pot.

Input is the yellow screened wire, output the green one.

Here you should connect a 500k log pot for Drive (Gain).

Here you connect a pushbutton for bypassing.
