The holes are 0.05" x 0.1", the annular rings are 0.1" x 0.15".

not that I'm aware of, but Buggs had a blast of a bunch of things posted recently, soo will be working on all of the documentation, which will include the schematics between now and the end of the year over the holidays!

It's a Sallen-Key filter. A 2 pole filter that in this case is acting as a low pass. Then can be set to be unity or amplify. The op-amp big muff has one that's set up has an amplifying low pass filter.
